In 1982, I was asked by a general practitioner to urgently see a 72-year-old man who earlier that day had tried to break down a door at the back of his house in order to escape a perceived intrusion by armed men. It was our habit then to see most new psychiatric referrals at home typically accompanied by medical students who enjoyed escaping the hospital for a ride in my car. His wife told me that her husband had not been himself since retirement from a manual job 5 years earlier. Never a talkative man, he had become increasingly quiet and withdrawn with little interest in his garden or other activities. He had also become physically slower and spent several hours a day sleeping in his chair if left undisturbed. His wife wondered if he was depressed, but since he made no complaints and his mood did not seem low, they sought no help at that stage .For the last year, he had started to have episodes of confusion in which he appeared not to be fully orientated and spoke about seeing people in his house when his wife was clear that nobody was there. In between these episodes, he was more forgetful than previously although this was variable and at times he seemed almost his normal self. Over the last 2 months, he had repeatedly talked about seeing soldiers hiding in the rosebushes in his garden. These frightened him, and it was in response to thinking that they were about to come into his house and shoot him and his wife that he made the escape attempt that led to my visit.

Mental Health Economics

Health Economics (HE) emerged in healthcare in the 1950s but was only applied in Mental Health mainly after the 1990s. One of the reasons for such a delay was the previous approach to psychiatric care based exclusively in hospitals. With the shift from a hospital to community care model, and with the availability of new, expensive treatments and a growing demand for treatment, HE became a powerful tool to guide mental health policies and to assess the costs and benefts of mental health interventions. HE has its origins on the principles of the welfare theory, which aims to maximize welfare (well-being and happiness) underpinned by a cost-beneft analysis. Because methodologies measure health in monetary units, the health sector adapted economic principles to an extra-welfarist approach through cost-effectiveness and cost-utility analyses in order to maximize health outcomes and guide public health resource allocation. In this chapter we present the main concepts of HE and how economic principles can be applied in mental health care. Economic evaluation is the most important method to verify benefts over costs, demonstrating which of several alternatives is the best value for the money. Regarding some peculiarities of mental health care, this chapter raises some challenging issues of applying HE methods and how the social and economic burden of mental disorders exert influence on the type of economic evaluation chosen.

Modern Concepts of Peripheral Nerve Repair

Peripheral nerves arise from the spinal cord, and they contain axons from different types of neurons serving various effector organs or sensory endings. The cell bodies of motor neurons that innervate skeletal muscle fbres are situated in the anterior horns of the grey matter of the spinal cord. Enlargements of the cord in the cervical and lumbar segments mark the major regions supplying the upper and lower limbs (Fig. 1.1). The frst sensory neurons are situated in the dorsal root ganglia, which are located in the intervertebral foramina, just proximal to the fusion of the anterior and posterior roots. The peripheral nerve is composed of motor, sensory and sympathetic nerve fbres. A nerve fbre is the conducting unit of the nerve and contains the following elements: a central core, the axon and Schwann cells. Some nerve fbres are surrounded by a myelin component (myelinated nerve fbres), and others are free of such myelin sheath (unmyelinated nerve fbres) .

Extracellular vesicles are recently described as specialized structures for intercellular communication. Their role in the central nervous system was diffusely studied in both physiological and pathological condition. In particular, an increased extracellular vesicle number was detected in several autoimmune diseases, including multiple sclerosis, a chronic autoimmune, inflammatory, demyelinating and neurodegenerative disease. This chapter summarizes the available information on the involvement of the extracellular vesicles in multiple sclerosis pathogenesis and their possible use as biomarker of therapy effcacy.

Classifcation of lung adenocarcinoma was largely revised in the 4th edition of WHO classifcation of tumors of the lung, pleura, thymus, and heart published in 2015. This chapter deals with the major changes in the adenocarcinoma classifcation, briefly describing the defnition, gross and histopathological fndings, genetic profles and clinical features of each subtype, and variants of lung adenocarcinoma. Special reference was also made to the prognostic aspects. The new concepts of adenocarcinoma in situ and minimally invasive adenocarcinoma are especially important from the prognostic point of view because of their virtual connotation as 100% curable cancers if resected completely. Each subtype of invasive adenocarcinoma may be categorized into either good, intermediate, or poor prognostic group. Much progress has been made regarding the genetic profles as well, such as the occurrence of EGFR and KRAS mutations, ALK fusion genes and recently discovered alterations, and NRG1 fusion genes in association with adenocarcinomas with certain characteristics. A brief overview of the major changes in the lung adenocarcinoma classifcation in this chapter will help physicians, radiologists, and pathologists grasp the signifcance and meaning of the histopathological diagnosis according to the new WHO classifcation.

Given the extensive history of cancer, the history of patient-derived xenograft (PDX) models is also diffcult to fully recapitulate. In particular, this task is complicated by the irregularity with which PDX models were designated. The current use of “PDX cancer models” is a relatively recent addition to the lexicon. However, the general concept of PDX models—i.e., the transplantation of human cancers into animal models—can be found throughout the chronicles of cancer research. However, it wasn’t until the discovery of host immunity and its crucial role in graft survival that the idea of serial transplantation could be realized. Therefore, in order to better reflect the nature of its history, the term “human tumor models” will be used in place of “PDX models,” except when appropriate. Finally, it is important to also mention the existence of the other labels that have been used, including, but not limited to, “human tumor xenografts,” “xenopatients,” heterotransplant tumor models,” “heterotransplanted human tumors,” and “transplantable tumor models.”

Reoperative Parathyroid Surgery

As with most surgical procedures, a surgeon’s knowledge of the relevant anatomy is critical to the technical success of operations on the parathyroid glands. This knowledge must encompass more than just an understanding of the anatomy of the normal neck; it must also include an understanding of the variant anatomy and aberrant anatomic relationships that may be encountered. The process is reinforced by an appreciation and understanding of not only parathyroid anatomy but also parathyroid embryology. This chapter reviews the embryology and anatomy of the parathyroid glands in a clinically relevant manner. Furthermore, it details recent advances in our understanding of the molecular mechanisms of parathyroid development. Our aim is to reinforce the link between embryology and surgical anatomy in the context of parathyroid surgery such that parathyroid glands are successfully identifed and operative morbidity is minimized. We also intend for this to serve as a reference and preface to the other chapters in this text.
