ABSTRACT
Over the last 20–30 years, there have been signifcant advances in the management of children with anorectal malformations, both medical and surgical. Changes have also occurred in the terminology used to describe the various types of anorectal malformation. Previously, the terms “low,” “intermediate,” and “high” malformations were used; however, these terms are not as anatomically clear as those used in the Krickenbeck Classifcation, developed in 2005. It is vital that the pediatric surgical community speaks the “same language” around the world, because having the correct diagnosis from the outset ensures that correct decisions can be made with regards to surgical planning and, ultimately, operative repair. In addition, accurate anatomic classifcation helps with the prediction of functional outcome. Failure to recognize the type of malformation can lead to adverse results. Therefore, we must ensure that data are reported in a standardized, uniform way in order for results to be comparable.
INTRODUCTION
Children with anorectal malformations often have impaired functional outcomes. It is recognized that the following features are likely to impact the likelihood of the child achieving fecal continence: • Type of anorectal malformation • Sacral ratio • Associated spinal anomalies Individual components have been studied and reported in the literature. The extent to which all of these factors interact with each other (ARM continence index) is not known, but is currently under investigation. With the development of this index, we believe that our ability to predict the likelihood of fecal continence, even prior to surgical intervention, will be improved, and that this will serve as an important guide when counseling families and setting expectations for the future. In addition, benchmarking outcomes based on a set of parameters (type of ARM, sacral ratio and spinal anomalies) is potentially an important tool to ensuring minimum quality standards and improving the quality of care overall.
چکیده
