ABSTRACT
Gaining a working knowledge of the equine gastrointestinal tract and associated intra‐abdominal organs can be challenging, especially for inexperienced individuals. Experienced veterinarians who examine and treat horses with conditions characterized by acute abdominal pain (colic) know that the key to the diagnosis often lies in recognizing changes in anatomic structures or relationships among different organs. With this in mind, the focus of this chapter is the gross and microscopic structure of the horse’s alimentary tract (Figure 1.1A, B, C ,and D), starting with the esophagus. Because some conditions characterized by colic involve other organs within the abdomen, we have reviewed the relevant structural aspects of the liver, spleen, and pancreas. In compiling this information, our goal is to provide veterinary students and veterinarians with the foundational materials needed to understand clinical conditions that result in colic.
INTRODUCTION
The esophagus is the long muscular tube that connects the pharynx to the stomach. It is regionally subdivided into cervical, thoracic, and abdominal parts. Individual and breed variations exist, but in general the esophagus is positioned on the dorsal aspect of the trachea at the level of the 1st cervical vertebra, inclines to the left lateral surface of the trachea at the level of the 4th cervical vertebra, and is positioned ventrolateral to the trachea from the level of the 6th cervical vertebra up to and during passage through the thoracic inlet. The thoracic portion of the esophagus travels within the mediastinum and is positioned dorsal to the trachea to the level of the tracheal bifurcation. The esophagus passes dorsal to the base of the heart and continues caudally until it penetrates the diaphragm at the esophageal hiatus, accompanied by the dorsal and ventral vagal trunks. The abdominal portion of the esophagus is short and travels over the dorsal border of the liver, creating an esophageal impression, before joining the cardia of the stomach at an acute angle. The esophagus is more superficial and therefore more accessible for surgery in the mid‐ to caudal‐third of the left side of the neck ventromedial to the jugular groove. Deep cervical fascia ensheathes the esophagus as it passes along the neck and also forms the left carotid sheath enclosing the left common carotid artery, the left vagosympathetic trunk, and the left internal jugular vein (when present). These structures, along with the neighboring left recurrent laryngeal nerve and the left tracheal lymphatic trunk (embedded within the deep cervical fascia that ensheathes the trachea), are to be avoided during surgical approaches to the esophagus.
چکیده
بدست آوردن دانش کاری در مورد دستگاه گوارش معجزه آسا و ارگان های داخل شکمی می تواند به ویژه برای افراد بی تجربه چالش برانگیز باشد. دامپزشکان متخصص که بررسی و درمان اسب با شرایط مشخص شده با درد حاد شکم (کولیک) می دانند که کلید تشخیص اغلب در به رسمیت شناختن تغییرات در ساختارهای تشریحی یا روابط بین ارگان های مختلف است. با توجه به این موضوع، تمرکز این فصل، ساختار ناخالصی و میکروسکوپی در دستگاه گوارش اسب است (شکل 1.1A، B، C و D)، شروع به مری می شود. از آنجایی که برخی از شرایطی که توسط کولیک مشخص شده اند شامل اندام های دیگر در داخل شکم هستند، ما جنبه های ساختاری مرتبط با کبد، طحال و پانکراس را بررسی کرده ایم. در جمع آوری این اطلاعات، هدف ما این است که دانش آموزان دامپزشکی و دامپزشکان را با مواد پایه ای مورد نیاز برای درک شرایط بالینی که باعث کولیک می شوند، بدانیم.
مقدمه
مری لوله ی عضلانی طولانی است که حلق را به معده متصل می کند. این منطقه به صورت منطقه ای به قسمت های گردن، قفسه سینه و شکم تقسیم می شود. تغییرات فردی و نژادی وجود دارد، اما به طور کلی، مری در قسمت پشتی قفسه سینه در سطح مهره ی گردن رحم قرار دارد، به سمت سمت چپ لگن در سطح مهره ی چهارم گردن رحم قرار می گیرد و موقعیت دارد از طریق سطح شریانی مهره های گردنی به داخل و خارج از طریق ورودی قفسه سینه، به سمت داخل تراشه حرکت می کند. بخش قفسه سینه مرفین در داخل مدیاستین حرکت می کند و به عقب به سمت تراشه به سطح بی اختیاری تراشه قرار می گیرد. مری در سمت راست قلب به عقب رانده می شود و تا زمانی که نفوذ به دیافراگم در انسداد مری و همراه با تنه پشتی و شکمی ایجاد می شود، ادامه می یابد. قسمت شكم مری كوتاه است و از مرز پشتی كبد عبور می كند و باعث ایجاد مرضی می شود، قبل از پیوستن به قلب معده در زاویه حاد. مری به صورت سطحی بیشتر است و از این رو برای عمل جراحی در قسمت سوم از سمت چپ گردن ونتومدیال به شیار جگوولار قابل دسترس است. فسیکا عمیق سرویکس، مری را در طول گردن حمل می کند و همچنین پوست غده کاروتید را که حاوی شریان کاروتید مشترک چپ، چپ وگوسیمپاتیک چپ و ورید ژوگولار سمت چپ است (در صورت وجود) تشکیل می دهد. این ساختارها همراه با عصب حنجره ای مجاری چپ مجاور و چانه لنفاوی تراشه چپ (که در داخل فاسیای عمقی گردن رحم قرار گرفته اند که باعث تنگی نفس می شوند) باید اجتناب شود
