ABSTRACT
Blood is the essential transport system that enables life in large multicellular, multiorgan animals. Without blood flow, there is no life, and arguably the most important element being transported is oxygen, which requires hemoglobin contained within red blood cells (RBCs). The ability to deal with infection and make an adequate inflammatory response to challenges depends on the white cell fraction of blood, which can transition rapidly into specific tissues as required. The requirement for blood to flow, and hence remain fluid, requires significant infrastructure and mechanisms to deal with inevitable breaches of the vascular endothelium. The function of the hemostatic system will be dealt with in Chapter 2. Diseases of almost every other organ can have an impact on blood, and conversely, diseases of blood can produce symptoms related to almost every other organ system. Understanding normal hematopoiesis and the physiology of normal blood is therefore essential to all healthcare professionals.
INTRODUCTION
In the early weeks of embryonic life, blood cells are produced within the yolk sac. From approximately 6 weeks through 7 months of gestation, the predominant sites of blood production are the liver and spleen. The bone marrow becomes the main production site of blood from 7 months of gestation onward. Initially, in infants, the active bone marrow is located throughout most axial and long bones of the skeleton, but there is progressive fatty replacement of marrow throughout life, with constriction of the active bone marrow toward the axial skeleton. By early adult life, active bone marrow is mostly restricted to the skull, sternum, vertebrae, ribs, and pelvis. Even in sites of active hemopoiesis, the bone marrow consists of approximately 50 percent fat spaces. Active bone marrow has a relatively high blood flow and is functionally in continuity with blood.

خون سیستم حمل و نقل ضروری است که زندگی را در حیوانات چند سلولی و چندگانه بزرگ فراهم می کند. بدون جریان خون، زندگی وجود ندارد و مسلما مهمترین عنصر منتقل شده اکسیژن است که نیاز به هموگلوبین موجود در سلولهای قرمز خون دارد (RBCs). توانایی مقابله با عفونت و پاسخ التهابی مناسب به چالش ها بستگی به کسر سلول های سفید خون دارد که می تواند به سرعت به بافت های خاص منتقل شود. نیاز به جریان خون و از این رو مایع باقی می ماند، نیاز به زیرساخت های قابل توجه و مکانیزم برای مقابله با نقص های اجتناب ناپذیر اندوتلی عروق دارد. عملكرد سيستم هموستاتيك در فصل دوم مورد بررسي قرار ميگيرد. بيماريهاي تقريبا هر ارگان ديگر ميتوانند بر خون اثر داشته باشند و برعكس، بيماريهاي خوني ميتوانند علائم مربوط به تقريبا هر سيستم عضو ديگر را تشكيل دهند. شناختن خونریزی طبیعی و فیزیولوژی خون طبیعی به همین دلیل برای تمام متخصصان مراقبت های بهداشتی ضروری است.
مقدمه
در هفته های اولیه زندگی جنین، سلول های خونی در داخل کیسه زرده تولید می شوند. از حدود 6 هفته تا 7 ماه حاملگی، سایت های غالب تولید خون کبد و طحال هستند. مغز استخوان به عنوان محل اصلی تولید خون از 7 ماهگی بارداری به بعد تبدیل می شود. در ابتدا، در نوزادان، مغز استخوان فعال در بسیاری از استخوان های محوری و طولانی اسکلت قرار دارد، اما جایگزینی چربی در مغز در طول زندگی وجود دارد، با سفت شدن مغز استخوان فعال به سمت اسکلت محوری. با رشد زودرس بزرگسال، مغز استخوان فعال عمدتا به جمجمه، سینه، مهره، دنده ها و لگن محدود می شود. حتی در سایت های هموپوئیدی فعال، مغز استخوان شامل تقریبا 50 درصد فضای چربی است. مغز استخوان فعال دارای جریان خون نسبتا بالایی است و به طور پیوسته با خون کار می کند.
