ABSTRACT
Neuroblastoma is a predominantly pediatric cancer, arising from the primordial neural crest cells that form the sympathetic nervous system. The prognosis for patients with neuroblastoma can vary from uniform survival in low risk patients to fatality in patients with high risk disease. This chapter gives a brief overview of the epidemiology, genetics, clinical presentation, diagnosis, and discussion of the various staging systems and risk classifcations of neuroblastoma. We also briefly describe our understanding of the conventional and novel treatment modalities available and their effects on the current prognosis of patients with neuroblastoma. The purpose of this chapter is to serve as a brief overview of the clinical aspects of neuroblastoma, to serve as a foundation of knowledge for scientists aspiring to develop new therapeutic modalities for this dreadful pediatric disease.
INTRODUCTION
Neuroblastoma is a neoplasm arising from the primordial neural crest cells that form the sympathetic nervous system. It is a cancer predominantly seen in pediatric patients. It is the mostcommon extracranial malignancy of childhood and the most common solid tumor of infancy . It is one of the most enigmatic tumors with extremely heterogeneous clinical behavior that ranges from spontaneous regression to metastatic disease refractory to therapy. It accounts for about 7% of all childhood malignancies; however 10% of childhood cancer related mortality. Treatment approaches have been based on presence or absence of specifc clinical or biologic factors. Although substantial progress has been made in the treatment and outcomes for low and intermediate risk neuroblastoma, success in therapy of high risk neuroblastoma remains evasive and complicated. Patients with low and intermediate risk neuroblastoma have an overall survival rate exceeding 90% and are now moving toward minimization of therapy . However in spite of standard therapy for high risk neuroblastoma patients involving multiagent chemotherapy induction, surgery and external beam radiotherapy, myeloablative chemotherapy with autologous hematopoietic stem cell rescue and biologic agents, only 50% of patients with newly diagnosed high risk disease will survive.
چکیده
نوروبلاستوما، سرطانی است که عمدتا در کودکان ایجاد می شود که از سلول های خاردار عصبی اولیه تشکیل شده اند که سیستم عصبی سمپاتیک را تشکیل می دهند. پیش آگهی بیماران مبتلا به نوروبلاستوما می تواند از بقای یکنواخت در بیماران کم خطر تا مرگ و میر در بیماران مبتلا به بیماری های پر خطر متفاوت باشد. این فصل خلاصه ای از اپیدمیولوژی، ژنتیک، ارائه بالینی، تشخیص و بحث در مورد سیستم های مختلف ارائه و طبقه بندی های خطر neuroblastoma را ارائه می دهد. ما همچنین درک ما از روش های معمول و جدید درمان در دسترس و آنها را در پیش بینی فعلی بیماران مبتلا به نوروبلاستوم توصیف می کنیم. هدف از این فصل این است که به عنوان یک مرور کلی از جنبه های بالینی نوروبلاستوم خدمت کنیم تا به عنوان پایه ای از دانش برای دانشمندان در جهت توسعه روش های درمانی جدید برای این بیماری کودک ترسناک خدمت کنیم.
مقدمه
نوروبلاستوما نئوپلاسم ناشی از سلول های خاردار عصبی اولیه است که سیستم عصبی سمپاتیک را تشکیل می دهند. این سرطان است که عمدتا در بیماران کودکان دیده می شود. این بدخیمی بیش از حد معمول دوران کودکی و شایع ترین تومور سالم کودکی است. این یکی از غم انگیزترین تومورها با رفتار بالینی ناهمگن است که از رگرسیون خودبهخودی تا بیماری متاستاز مقاوم به درمان است. این حدود 7 درصد از تمام بدخیمی های دوران کودکی را شامل می شود؛ با این حال 10 درصد مرگ و میر مربوط به سرطان دوران کودکی است. رویکردهای درمان براساس وجود یا عدم وجود مشخصه های بالینی یا عوامل زیست شناختی است. اگر چه پیشرفت های قابل توجهی در درمان و نتایج برای نوروبلاستوم خطر پایین و متوسط وجود دارد، موفقیت در درمان neuroblastoma با ریسک باالیی باقی می ماند و پیچیده است. بیماران مبتلا به نوروبلاستوما با ریسک کم و متوسط دارای میزان بالایی از بقا هستند که بیش از 90٪ هستند و در حال حاضر به سوی کمینه کردن درمان حرکت می کنند. با این حال به رغم درمان استاندارد برای بیماران نوروبلاستوم در معرض خطر نیز از جمله القای multiagent شیمی درمانی، جراحی و پرتو درمانی خارجی، شیمی درمانی myeloablative با اتولوگ خون ساز امداد و نجات سلول های بنیادی و عوامل بیولوژیک، تنها 50٪ از بیماران مبتلا به بیماری در معرض خطر تازه تشخیص داده شده زنده خواهد ماند.
