ABSTRACT
The pioneer of total hip replacement (THR), John Charnley, introducing his new method, wrote 55 years ago: “Neither surgeons nor engineers will ever make an artifcial hip joint which will last 30 years”. His prediction was not confrmed. This revolutionary method, low-friction arthroplasty (LFA), has already survived more than 40 years, while newer methods of THR promise even better results, although there is not yet a universal agreement as to which method of fxation of a THR, cemented, uncemented or hybrid, has the longest survival prospects. THR is considered as the operation of the century; however, we should not forget that it may involve unexpected disappointment. Therefore, it is of great importance to be decided when there is an absolute indication for THR, based on patients’ symptoms, psychological impact, clinical fndings and the potential of development of the disorder in each case. Orthopaedic surgeonsshould protect this operation from the overuse. The patients should be informed for the severity of the procedure, the possible complications and the necessary postoperative adaptations of their daily life. Although some THRs present several risk factors of a non-favourable outcome, related to the patient or the surgical technique, they may survive more than 35 years. This is an indication that we cannot safely predict the outcome of a THR with the known data. Constant follow-up remains the most reliable way to detect early signs of failure and application of early intervention, before extensive bone damage makes revision diffcult, necessitating more complicated surgical techniques.
INTRODUCTION
Before the introduction of THR, the treatment of choice in adults with CHD was intertrochanteric osteotomy. During these years, osteotomies were mostly performed without clear indication, and their results were not always satisfactory. The best results and the longest postponement of most radical procedures, such as THR, were obtained with the varus intertrochanteric osteotomy, in the early stages of the development of OA in dysplastic hips, and the valgus intertrochanteric osteotomy in a later stage. Recent years, osteotomies in adults with CHD are rarely performed. For that reason, surgeons usually have limited experience in this type of operations and should be decided with caution.

پیشگام کلسترول جایگزین (THR)، جان چارلی، معرفی روش جدید او، 55 سال پیش نوشت: “نه جراحان و نه مهندسان هرگز نمیتوانند مفصل مفصلی را که 30 سال به طول انجامد، ایجاد کنند.” پیش بینی او ثابت نکرد. این روش انقلابی، آرتروپلاستی با کمترین اصطکاک (LFA)، بیش از 40 سال باقی مانده است، در حالیکه روشهای جدیدتر THR وعده داده شده حتی نتایج بهتر را می دهد، اگر چه توافق جهانی وجود ندارد که کدام روش fxation THR، سیمان، بدون سمیت یا ترکیبی، دارای طولانی ترین چشم انداز بقا است. مقدمه
قبل از معرفی THR، درمان انتخاب در بزرگسالان مبتلا به CHD، استئویتومی اینترروچانتریک بود. در طی این سال ها، استئوتومی ها بدون هیچ نشانه ای مشخص انجام شد، و نتایج آنها همیشه رضایت بخش نبود. بهترين نتايج و طولاني ترين تحريم بيشتر روش هاي راديولوژيکي مانند THR با استئوآرتريت ورتوس ميتراچانتريک، در مراحل اوليه رشد OA در ناحيه هيپوفيزيک پلاسميدي و استئوتومي ميان ترشحي والگوس در مرحله بعد حاصل شد. سال های اخیر، استئواتومی در بزرگسالان مبتلا به CHD به ندرت انجام می شود. به همین دلیل، جراحان معمولا تجربه محدودی در این نوع عملیات دارند و باید با احتیاط تصمیم گیری شوند.
