Unusual Cases in Peritoneal Surface Malignancies

ABSTRACT

Crohn’s disease (CD) is a clinical condition characterized by inﬂammation of the intestines that may involve the entire thickness of the intestinal walls. CD most often involves the small intestine and colon. Over the past decades, intestinal adenocarcinoma has been recognized as a complication of CD. The management of patients with peritoneal metastasis (PM) of small bowel adenocarcinoma (SBA) associated with Crohn’s disease (CD) has not been defned yet. This report will be structured as a case report of PM of SBA associated with CD. From this data, other peritoneal surface oncology centers may wish to offer CRS and HIPEC to patients with similar features at early stages of this disease.

INTRODUCTION

Small bowel adenocarcinoma (SBA) has been found to be associated with Crohn’s disease (CD). In 1956, SBA associated with CD has been reported by Ginzburg for the frst time . Since then, there are numerous studies that have published the occurrence of small bowel carcinoma developed from CD. The relative risk of developing small bowel carcinoma in patients with CD was 28.37 in 9,642 patients . The mean duration of CD before the onset of adenocarcinoma was 9 (range 0.8–41) years . Risk factors for small bowel are listed in Table 1.1. The most common clinical presentation of SBA associated with CD is obstruction with nausea, vomiting, and abdominal pain. Other presentations are hemorrhage, fstula, and perforation . These symptoms are commonly seen in CD’s exacerbation, and it is almost always diffcult to differentiate small bowel adenocarcinoma from those of the CD’s activation. Majority of the patients with SBA associated with CD are diagnosed at the time of operation or postoperatively and only less than 5% cases are diagnosed preoperatively . Two indicators of the malignancy in patients with CD are exacerbation of the symptoms after long periods of the quiescence and small bowel obstruction refractory to medical treatments . Therefore, it is important to consider surgical assessment of patients with long-standing symptomatic CD who failed to respond to conservative treatments.

بیماری کرون (CD) یک وضعیت بالینی است که در اثر جذب روده ای که ممکن است کل ضخامت دیواره روده را شامل می شود، مشخص شود. سی دی اغلب شامل روده کوچک و روده بزرگ است. در دهه های گذشته، آدنوکارسینوما روده ای به عنوان یک عارضه سی دی شناخته شده است. مدیریت بیماران مبتلا به متاستاز صفاقی (PM) آدنوکارسینوم روده کوچک (SBA) مرتبط با بیماری کرون (CD) هنوز مشخص نشده است. این گزارش خواهد بود به عنوان گزارش مورد PM از SBA مرتبط با سی دی ساخت. از این داده ها، دیگر مراکز انکولوژی سطح پریتونال ممکن است مایل به ارائه CRS و HIPEC به بیماران با ویژگی های مشابه در مراحل اولیه این بیماری باشد.

مقدمه

آدنوکارسینوم روده کوچک (SBA) با بیماری کرون ارتباط دارد (CD). در سال 1956، SBA همراه با CD توسط Ginzburg گزارش شده است. از آن زمان مطالعات متعددی منتشر شده است که نشان می دهد وقوع کارسینوم روده کوچک از سی دی منتشر شده است. خطر نسبی کارسینومای روده کوچک در بیماران مبتلا به CD 37/28 بود در 9642 بیمار. متوسط مدت CD قبل از شروع آدنوکارسینوما 9 (محدوده 0.8-41 سال) بود. عوامل خطر برای روده کوچک در جدول 1.1 آمده است. شايع ترين نظر بالينی SBA مربوط به CD انسداد با تهوع، استفراغ و درد شکمی است. ارائه های دیگر خونریزی، فیستول و سوراخ شدن است. این علائم معمولا در تشدید CD دیده می شود و تقریبا همیشه برای تشخیص آدنوکارسینومای روده کوچکی از فعالیت های CD استفاده می شود. اکثر بیماران مبتلا به SBA مرتبط با CD در زمان عمل یا پس از عمل تشخیص داده می شوند و تنها کمتر از 5٪ موارد قبل از عمل تشخیص داده می شوند. دو شاخص از بدخیمی در بیماران مبتلا به CD علائم تشدید علائم پس از مدت زمان طولانی از خستگی و انسداد روده کوچک مقاوم به درمان های پزشکی است. بنابراين مهم است که ارزيابی جراحی بيماران مبتلا به سندرم دائمی علامت دار که قادر به پاسخگویی به درمان های محافظه کار نباشند، مورد توجه قرار گيرد.
