Dr. Lane is a new graduate and has entered into the field of hospital medicine. During her limited time as a hospitalist, it has become clear to Dr. Lane that she has a gap in her working knowledge of the healthcare system as a whole. During residency, she focused most of her attention on patient care and less on understanding the system in which she works. She is also starting to realize the impact the current healthcare system has not only on her patients but on herself as a healthcare provider. Understanding the basics of healthcare and the healthcare systems in which you work is an important aspect of being a successful hospitalist. This chapter will aim to provide a brief overview of the U.S. healthcare system and introduce you to the different aspects of the Affordable Care Act.

Clinical Approaches to Hospital Medicine

Heart failure (HF) affects more than 5.7 million adults in the United States and current projections estimate the prevalence of HF will continue to increase. Accounting for more than 1 million admissions annually, HF is the leading cause of hospitalization among the Medicare age group with an overall 1-year mortality rate of 29.6% . In 2013, total cost for HF was estimated to be $30.7 billion with 68% being attributable to direct medical costs. Projections show that by 2030, the total cost of HF will increase almost 127% to $69.7 billion with an estimated $244 spent annually for every US adult . As an emerging issue in hospital care, the hospitalist provider can anticipate a large portion of admissions with either primary or comorbid HF. In this article, we will discuss: • How to recognize HF and classify accordingly • Key treatment modalities • When to consult subspecialists • Risk factors for re-admission and strategies for prevention

High Performance in Hospital Management

What can we learn from other health systems? – What do the different stakeholders expect from their health system? – How innovative is your hospital? – Which managerial attitudes are not sustainable? This chapter introduces different health systems and health care costs. Over the past few years a new market-driven orientation of hospitals has taken place. We elaborate on the Kutzin framework to improve effciencies of health systems and highlight what the different stakeholders can anticipate with regard to a health system. In addition, the expectations of health care users in developing and developed countries are debated. The chapter ends with a questionnaire to investigate how innovative your hospital is and how you can engage in the innovation process.

Space or no space for managing public hospitals; a qualitative study of hospital autonomy in Iran

Iran is a lower middle-income country with a population of over 70 millions (World Bank, 2007). Total expenditure on health in 2005 is estimated to be over 6% of GDP, and general government expenditure on health is about 52% of total health expenditure (WHO, 2007). The health system in Iran is organized into three levels. Specialty and super-specialty healthcare services (upper level) are mainly located in big cities. The other levels (district general hospitals and primary healthcare networks) are predominantly in small towns, semi-urban, rural, and remote areas (Regional Health Systems Observatory, 2002). The legislation for structure and duties of Ministry of Health and Medical Education (MOHME) was approved in 1988. Accordingly, faculties of medical sciences were detached from traditional universities, establishing the niversities of Medical Sciences and Health Services at the provincial level under the supervision of MOHME
