AKT, also known as protein kinase B and RAC-PK, was first discovered as an oncogene transduced by the acute transforming retrovirus (AKT-8), which is known to cause leukemia in mice. AKT is the major downstream target of phosphatidylinositol 3-kinase (PI3K), which can be activated by receptor tyrosine kinases in response to various growth factors. AKT is a serine/threonine kinase located at the apex of a cascade of signaling pathways. Deregulated AKT signaling is implicated in cancer cell growth, proliferation, and survival. Novel antitumor strategies have now been developed to target AKT and key downstream targets in the clinic.

Patient Involvement in Health Technology Assessment

This chapter will explore the history of health technology assessment (HTA) and how it has evolved in terms of processes and methods, highlighting elements relevant to patient involvement to lay a foundation for the subsequent chapters of this book. HTA is a policy analysis that seeks to inform decision-makers in national, regional or hospital health services about the use of health technologies. HTAs require systematic processes that critically assess research about the impacts of using the health technology along with context-specifc appraisal of the social, economic, legal and ethical implications of the use of the health technology. This is not simply a scientifc endeavour. It requires interdisciplinary deliberative discussion and value judgements about the relevance of the evidence for the local health system. HTAs may recommend the use or disinvestment of a health technology and so are subject to political, public and stakeholder scrutiny. As a result there has been pressure to involve those who have a specifc interest in the health technology, particularly patients, in the TA process. However, this is contentious due to concerns about potential bias and representativeness of patient input and the scientifc integrity of patient evidence.

This book outlines our unique conceptualization of perfectionism, as well as assessment and treatment approaches for it. It represents the culmination of over two decades of research and clinical work on this topic. Our primary purpose for writing this book is to offer insight into the complex construct of perfectionism—not only as a personality style involving traits and relational elements, but also as a clinically relevant personality vulnerability factor that predisposes individuals to myriad problems. We also provide information on appropriate assessment and treatment of those people who are paying a signifcant personal price for their perfectionism.

Perineal Trauma at Childbirth

Review of historical literature confrms that perineal injury has occurred during childbirth throughout the ages and that various methods and material were used by accoucheurs in an attempt to restore the integrity of severely traumatised tissue. Perineal stitching following childbirth was advocated in ancient writings on midwifery and obstetrics, however the procedure was not routinely practiced. Women remained the prominent fgure during confnements in the early centuries and male physicians or barber-surgeons were only called in as a last resort if problems occurred. During the eighteenth century the introduction of forceps together with episiotomy to facilitate diffcult deliveries had a major impact on the extent of perineal trauma and its subsequent repair. Furthermore, women were encouraged to deliver in a more supine position rather than upright so the perineum was more accessible and the full extent of perineal trauma sustained could be assessed. During this period more attention was made to minimising perineal trauma and various methods including supporting the perineum and applying pressure to the vertex to prevent rapid expulsion were implemented. By the end of the nineteenth century practitioners were advised to suture all perineal trauma. However, it was not until the early twentieth century that local anaesthetic was advocated to ease the pain prior to performing and suturing episiotomies. In the UK, midwives did not receive any formal training until the late eighteenth century and it was not until the late twentieth century that midwives were permitted to undertake perineal suturing.

Performance assessment of food safety management system in the pork slaughter plants of China

The aim of this study was to investigate the status of food safety management system (FSMS) implemented at the pork slaughter plants in China, based on a detailed profile of both announced assessments carried out in 60 companies and unannounced assessments in 25 ones, with the checklist covered 29 indicators. The results from the study indicated several factors had an effect on the status of FSMS, associated with company size, location, target market, and valid certificates. The results also revealed a weakness of FSMS on good manufacture practices, including contamination control, pest control, clean and disinfection, facility environment and personal hygiene. In order to precisely measure implementation performance and better identify insufficiency, unannounced assessment was shown to be more efficient than announced assessment.
