ABSTRACT
An estimated 8–12 million Americans are believed to suffer from peripheral arterial disease (PAD) according to the American Heart Association. The prevalence of PAD has been estimated at almost 10% in the general population and almost 20% in those older than 70 years. Patients with PAD present a unique challenge to the provider due to their typically older age, high rates of underdiagnosis due to asymptomatic state, and the higher prevalence of comorbid conditions. The prototypical risk factors for PAD are similar to that of coronary arterial disease and include tobacco use, hyperlipidemia, hypertension, and diabetes. The mainstay of PAD therapy is aggressive risk factor modifcation followed by pharmacologic therapy and exercise therapy as warranted.
INTRODUCTION
Though cigarette smoking is quite well known as risk factor for other cardiovascular conditions, it has the absolute strongest correlation with PAD. Tobacco use is the largest risk factor for PAD with epidemiological tudies demonstrating up to a sixfold increased risk of the development of PAD in patients who smoke tobacco; furthermore, this risk has been demonstrated to be dose dependent on the amount of tobacco use . Moreover, once a patient has been diagnosed with PAD, the continued use of cigarettes has been directly related to the progression of PAD and eventual limb loss. This effect is also seen in an attenuation of response with the use of pharmacologic and endovascular interventions in the PAD patient who continues to use cigarettes . Studies have also demonstrated an increased dose-dependent risk of PAD development in patients who do not smoke cigarettes themselves but are a victim of secondhand smoke exposure . More recently, e-smoking has been increasing in popularity due to perceived lower-risk profle; though conclusive studies on e-smoking’s risk on PAD do not exist, there are some concerns that the perceived benefts of e-smoking may be outweighed by greater vapor and secondhand exposure in regard to PAD risk.

به گفته انجمن قلب آمریکا، حدود 8-12 میلیون آمریکایی مبتلا به بیماری شریانی محیطی (PAD) هستند. شیوع PAD در جمعیت عمومی تقریبا 10٪ و در افراد بالای 70 سال تقریبا 20٪ تخمین زده شده است. بیماران مبتلا به PAD با توجه به سن آنها به طور معمول، سن بالا و پایین بودن تشخیص به علت وضعیت بدون علامت و شایع بودن بیماری های همراه، یک چالش منحصر به فرد برای ارائه دهنده است. عوامل خطر ابتلا به PAD مشابه بیماری های شریان کرونر هستند و شامل مصرف دخانیات، هیپرلیپیدمی، فشار خون بالا و دیابت می باشند. اصلی درمان PAD، اصلاح عامل خطر تهاجمی است و به دنبال آن درمان دارویی و ورزش درمانی ضروری است.
