ABSTRACT
Sickle cell disease (SCD) and β-thalassemia are among the most common inherited diseases, affecting millions of persons globally. It is estimated that 5–7% of the world’s population is a carrier of a signifcant hemoglobin variant. Without early diagnosis followed by initiation of preventative and therapeutic care, both SCD and β-thalassemia result in signifcant morbidity and early mortality. Despite great strides in the understanding of the molecular basis and pathophysiology of these conditions, the burden of disease remains high, particularly in limited resource settings. Current therapy relies heavily upon the availability and safety of erythrocyte transfusions to treat acute and chronic complications of these conditions, but frequent transfusions results in signifcant iron overload, as well as challenges from acquired infections and alloimmunization. Hydroxyurea is a highly effective treatment for SCD but less so for β-thalassemia, and does not represent curative therapy. As technology and use of cellular and gene therapies expand, SCD and thalassemia should be among the highest disease priorities.
INTRODUCTION
Inherited disorders of hemoglobin, primarily sickle cell disease (SCD) andβ-thalassemia are among the most common monogenetic diseases in the world . The importance of these hematological disorders cannot be overstated; it is estimated that 5–7% of the world’s population are carriers of a signifcant hemoglobin variant. SCD has often been described as the “frst molecular disease” following Linus Pauling’s discovery in 1949 that the disorder was due to an abnormal hemoglobin molecule . This landmark report was followed several years later by a crucial elucidation by Vernon Ingram that sickle hemoglobin (HbS) is due to a single amino acid change in the β-globin chain of the hemoglobin molecule. These reports were the frst to describe the specifc molecular basis for a human disease, and led to a flurry of scientifc investigation of normal and abnormal hemoglobin, including several seminal fndings describing the genetic basis of the thalassemias.
چکیده
بیماری سلولی صرع (SCD) و β-تالاسمی یکی از شایع ترین بیماری های ارثی است که میلیون ها نفر را در سطح جهان تحت تأثیر قرار می دهد. برآورد شده است که 5-7٪ جمعیت جهان حامل یک نوع هموگلوبین قابل توجه است. بدون تشخیص زود هنگام و همچنین شروع مراقبت های پیشگیرانه و درمانی، هر دو SCD و بتا تالاسمی باعث مرگ و میر ناشی از مرگ و میر می شود. علیرغم پیشرفت های زیادی در درک مبانی مولکولی و پاتوفیزیولوژی این شرایط، بار بیماری همچنان بالا است، به خصوص در تنظیمات منابع محدود. درمان فعلی به شدت بر روی در دسترس بودن و ایمنی انتقال خون اریتروسیتها برای درمان عوارض حاد و مزمن این شرایط تکیه دارد، اما انتقال مکرر باعث انتقال بیش از حد آهن و همچنین چالش های عفونت های به دست آمده و آلوئونیزاسیون می شود. هیدروکسی اوره درمان بسیار مؤثر برای SCD است اما کمتر از آن برای β-تالاسمی و درمان سرپایی نیست. به عنوان تکنولوژی و استفاده از درمان های سلولی و ژنی، SCD و تالاسمی باید از بالاترین اولویت های بیماری باشد.
