ABSTRACT
Spine is a multiarticular system controlled by muscles supporting the head and trunk during posture and movements; it protects the spinal cord, nerve roots and, at cervical level, the vertebral arteries. The normal function of the spine presupposes its stability . Furthermore spine stability is the essential principle to transfer power forces between the upper and lower limbs and the active generation of forces in the trunk . Clinical stability is the spine’s ability under physiologic loads to limit patterns of displacement in order not to damage or irritate the spinal cord and nerve roots and to prevent incapacitating deformity or pain caused by structural changes . Vertebral stability during movements is obtained with bony and soft movement restraints. The loss of stability is an important cause of back pain; because spinal movements are not isolated
but coupled among them, tissue derangement frequently causes dysfunctional motions in multiple directions.
Spinal stability is based on three connected systems (Fig. 1.1): • Column • Muscles and tendons • Central nervous system and spinal nerves The column includes bones, discs, ligaments and joint capsules; these structures fulfl an intrinsic structural role and contain mechanoreceptors which act as transducers, sending a
continuous flow of proprioceptive information on loads, motions and posture through the spinal nerves to the central nervous system that, in turn, replies via an appropriate and coordinated feedback muscular action .
INTRODUCTION
The passive load-bearing ability of the vertebral body depends on the size, shape, integrity of the trabecular system and bone density. The vertebral body mainly consists of spongy bone with a three-dimensional honeycomb structure that yields the best strength/weight ratio . The progressive increase in body size downward in the spine is the only physiological answer to increasing weight loads . The cancellous bone of any vertebral body has four main trabecular systems (Fig. 1.2): • A vertical system extending between the endplates which accepts and transmits vertical loads • A horizontal system travelling in the posterior arch and joining the transverse processes • Two curved oblique systems, superior and inferior, starting from the endplates and crossing in the peduncles to end in the spinous and joint processes Their function is to withstand the horizontal shear stresses ensuring the neural arch to the body. Compared to spongy bone, the cortical bone presents much lower elasticity but is more resistant. The resistance of spongy bone also strongly depends on mineral density; indeed bone loss in osteoporosis results in a disproportionate exponential reduction of resistance: a bone loss of 25% leads to a reduction of resistance of about 50%.
چکیده
ستون فقرات سیستم چند قطبی است که توسط عضلات پشت سر و تنه در حین بدن و حرکات کنترل می شود. آن را از طناب نخاعی، ریشه های عصبی و در سطح رحم، رگ های مهره محافظت می کند. عملکرد طبیعی ستون فقرات، ثبات آن را پیش بینی می کند. علاوه بر پایداری ستون فقرات، اصل ضروری برای انتقال نیروهای قدرت بین اندام فوقانی و اندام و تولید نیروهای فعال در تنه است. پایداری بالینی توانایی ستون فقرات در بارهای فیزیولوژیکی برای محدود کردن الگوهای جابجایی است تا آسیب و یا تحریک نخاع و ریشه های عصبی و جلوگیری از ناتوانی جنسی و یا درد ناشی از تغییرات ساختاری را نداشته باشد. پایداری ستون فقرات در طی حرکات با محدودیت حرکت استخوانی و نرم بدست می آید. از دست دادن ثبات یک علت مهم درد پشت است. چون حرکات ستون فقرات جدا نیستند، اما در میان آنها متصل است، اختلال بافت اغلب باعث حرکات نامناسب در چند جهت می شود. پایداری ستون فقرات بر اساس سه سیستم متصل است (شکل 1.1): • ستون • عضلات و تاندون ها • سیستم عصبی مرکزی و اعصاب ستون فقرات ستون شامل استخوان، دیسک، لیگامان و کپسول مفصلی است. این ساختارها یک نقش ساختاری ذاتی را به وجود می آورند و حاوی گیرنده های مکانیکی هستند که به عنوان مبدل عمل می کنند و اطلاعات مداوم از اطلاعات پروتئوزیتی را در بارهای، حرکات و موقعیت از طریق اعصاب نخاعی به سیستم عصبی مرکزی ارسال می کنند که به نوبه خود از طریق بازخورد مناسب و هماهنگ با عضلات عمل است .
مقدمه
توان نفوذپذیری غیر فعال در بدن مهره بستگی به اندازه، شکل، یکپارچگی سیستم تراککول و تراکم استخوان دارد. بدن مهره ای عمدتا از استخوان اسفنجی با ساختار لانه زنبوری سه بعدی تشکیل شده است که بهترین نسبت قدرت و وزن را به دست می دهد. افزایش مترقی در اندازه بدن به سمت پایین در ستون فقرات، تنها پاسخ فیزیولوژیکی به افزایش وزن است. استخوان مجزا هر بدن مهره ای دارای چهار سیستم اصلی ترابوکالری است (شکل 1.2): • یک سیستم عمودی بین انتهایی که بارهای عمودی را می پذیرد و انتقال می دهد • یک سیستم افقی که در قوس خلفی حرکت می کند و به فرآیندهای عرضی می پیوندد • دو منحنی منحنی سیستم های برتر و پایین تر، از انتساب ها و عبور از ستون ها به منظور جلوگیری از فرایندهای درشت و مفصلی آغاز می شود. عملکرد آنها مقاومت در برابر فشار برشی افقی است که قوس عصبی را به بدن تحمیل می کند. در مقایسه با استخوان اسفنجی، استخوان کورتیک الاستیسیته کمتری دارد اما مقاوم تر است. مقاومت استخوان اسفنجی نیز به شدت به تراکم مواد معدنی بستگی دارد. در واقع از دست دادن استخوان در پوکی استخوان منجر به کاهش مقاومت نامتعادل می شود: از دست دادن استخوان 25 درصد منجر به کاهش مقاومت حدود 50 درصد می شود.
