Diagnostic Clusters in Shoulder Conditions

An accurate diagnosis is the foundation of Good Medical Practice. The inspiration for this work has been to pull together the diagnostic knowledge, skills, and wisdom gathered over generations by the orthopaedic fraternity practising common sense orthopaedics. The relevance and need of such common sense-based clinical practice has been amplifed by the pressures faced by the chaotic Internet information boom, easy access to radiology, corporatization of medical care, and the rapid pace of our life demanding binary answers to complex questions. In routine clinical practice, the diagnosis of a particular shoulder condition is best made on history, clinical examination, and special tests, usually with the help of radiological investigations. It is often the case that all these subcomponents are to be used in conjunction with each other as “clusters” rather than in isolation. Current textbooks are focused on physical examination techniques or radiological examination and do not take into account this cluster approach one needs to use in everyday practice. The purpose of this book is to bridge this gap and serve as a practical guide to diagnosis in modern day clinical practice.

Endocannabinoids and Lipid Mediators in Brain Functions

In this chapter, the neuroanatomical overlap in the distribution of orexin and endocannabinoid systems, as well as the functional interaction between leptindriven synaptic rewiring of orexinergic neurons and the orexin receptor-mediated activation of 2-arachidonoylglycerol synthesis, will be presented in the context of their role in the regulation of appetite, reward, sleep/wake, and analgesia. This chapter attempts to piece together what is known about this important cross talk and points out its potential therapeutic implications.

Fertility Preservation and Restoration for Patients with Complex Medical Conditions

Signifcant improvements in medical treatments have led to decreased morbidity and mortality among individuals with complex medical conditions. These individuals are living longer, fuller lives. Advances in reproductive medicine now allow individuals with complex medical conditions whose disease or its treatment may impair fertility, to undergo fertility preservation. Fertility preservation options are available for females and males of prepubertal and reproductive age. In this chapter, we summarize the current fertility preservation options available, including experimental fertility preservation techniques, for both genders.
