ABSTRACT
The coronary artery is the frst branch of the aorta and is divided into the left and right coronary arteries. The left main coronary artery is derived from the left coronary cusp and is divided into the left anterior descending artery (LAD) and left circumflex artery (LCX). The LAD is located in the anterior interventricular groove and supplies the anterior wall, septum, and apex. The branches of the LAD are septal perforating arteries and diagonal branches. The septal perforating arteries supply most of the septum, and the diagonal branches supply the lateral wall of the left ventricle. The LCX passes through the atrioventricular groove and supplies the left atrium, as well as most of the lateral and posterior walls of the left ventricle. The branches of the LCX are obtuse marginal branches, and approximately 30–40% of the sinoatrial nodal branch is derived from the LCX. The right coronary artery (RCA) is derived from the right coronary cusp; it runs along the right atriventricular groove and continues to the posterior nterventricular sulcus. The RCA supplies the right atrium, right ventricle, sinoatrial node, and atrioventricular node via several branches (conus, right ventricular wall, sinoatrial nodal, trioventricular nodal branch). At the distal portion of the RCA (i.e., the crux), it divides into two branches: the posterolateral and posterior descending arteries, which supply the inferior portion of the interventricular septum and apex. In more than 80% of cases, the RCA has posterior descending and posterolateral branches. The others are leftdominant systems, in which the LCX gives rise to posterolateral and posterior descending branches, or codominant systems, in which both arteries provide an equal supply (Fig. 1).
INTRODUCTION
Coronary blood flow is a phasic pattern; main arterial flow in the coronary artery occurs in diastole. During systole, contraction of the myocardium compresses the coronary microvessels, impedes arterial blood flow, and increases venous outflow. Coronary flow is determined by myocardial demand and blood supply. Major determinants of myocardial blood flow are heart rate, myocardial contractility, and myocardial wall stress (preload, afterload). Because coronary blood flow passes from the epicardium to the endocardium, the subendocardial area is susceptible myocardial ischemia. The pressure difference between the epicardial coronary artery and the left ventricle is important to maintain myocardial perfusion.
چکیده
شريان کرونر شاخه اي از آئورت است و به شریان کرونر سمت چپ و راست تقسيم شده است. شریان اصلی عروق کرونر سمت چپ از عصب کرونر سمت چپ گرفته شده است و به شریان نزولی قدامی چپ (LAD) تقسیم می شود و عروق کرونر سمت چپ (LCX) تقسیم می شود. LAD در شیار داخل شکاف قدامي قرار دارد و دیوار قدامي، سپتوم و آپکس را فراهم می کند. شاخه های LAD، شریان های سوراخ سوراخ و شاخه های مورب هستند. شریان های سوراخ سوراخ بیشتر از سپتوم را تامین می کنند و شاخه های قطر دیواره جانبی بطن چپ را فراهم می کنند. LCX از طریق شیار اتریواسترویچ عبور می کند و دهلیز سمت چپ، و نیز بیشتر دیواره های جانبی و خلفی بطن چپ را تامین می کند. شاخه های LCX شاخه های حاشیه ای مبهم هستند و تقریبا 30-40٪ از شعاع گره sinoatrial از LCX مشتق شده است. شريان کرونر راست (RCA) از کاسپاراسيون سمت راست مشتق شده است؛ آن را در امتداد شیار قرنیه راست قرار می دهد و به سوزن nterventricular خلفی ادامه می دهد. RCA دهلیز سمت راست، بطن راست، گره سینیواتوری و گره اتوریوژن مجاری را از طریق چند شاخه (conus، دیواره سمت بطن، nodal sinoatrial، branch nodal triventricular nodal) فراهم می کند. در قسمت ديستال RCA (به عنوان مثال، crux)، آن را به دو شاخه تقسيم مي کند: شريان هاي نزولي posterolateral و posterior که نواحي پاييني سپتوم متقابل و آپکس را فراهم مي کنند. در بیش از 80٪ موارد، RCA دارای شاخه های نزولی خلفی و پس زمینه ای است. دیگران سیستم های سمت چپ، که در آن LCX باعث ایجاد شاخه های نزولی posterolateral و posterior می شود، یا سیستم های متداول که در آن هر دو شریان ها یک عرضه مساوی را ارائه می دهند (شکل 1).
مقدمه
جریان خون کرونری یک الگوی فازی است؛ جریان اصلی شریانی در عروق کرونر در دیاستول رخ می دهد. در طی سيستول، انقباض ميوکارد، عروق کرونر را فشرده می کند، مانع جریان خون شریانی می شود و جریان خون وریدی را افزایش می دهد. جریان کرونری توسط نیاز قلب و خون تامین می شود. مهمترین عوامل تعیین کننده خونریزی قلب، ضربان قلب، انقباض قلب و استرس دیواره میوکارد (پیش بارگذاری، پس از بارگذاری) می باشد. از آنجایی که جریان خون کرونری از epicardium به endocardium عبور می کند، ناحیه زیر اندوكاردی حساس به ایسکمی میوكارد است. اختلاف فشار بین شريان عروق کرونر و بطن چپ برای حفظ پرفيوژن قلب ضروری است.
