Tears are essential for the normal function of the eye. A part of the tears is lost by evaporation. The majority of tears drain to the inferior meatus of the nose. The parasympathic nervous system controls the tear volume reflex by the ffth cranial nerve. When the volume increases or the passage is obstructed, the patient complains about epiphora and blurred vision. Bacterial invasion of an obstructed lacrimal system can occasionally lead to acute dacryocystitis with fstula formation. The patient should be informed that in almost every case (except for orbital abscess) the operation is elective and optional. To choose the correct therapy a careful history nd an examination of the eyelid and the lacrimal system are necessary.

Vascular Surgery

In this chapter, we will briefly outline some of the current initiatives in global surgery that focus on surgical care for vulnerable populations primarily in low and middle-income countries (LMICs). In recent years, global disparities in surgical access and outcomes have gained greater attention. In 2015 specifically, a number of related initiatives have launched and may provide a template for further work in various surgical specialties. In this chapter, we will outline some of these initiatives within the context of global health initiatives, discuss models for global engagement, and propose possible areas of consideration to increase vascular surgery capacity in resource- poor areas. In the past 15 years, global health initiatives have been led by the eight United Nations Millennium Development Goals (MDGs), with several of these goals impacted by treatment of surgical conditions . As the time frame of the MDG’s have come to an end, significant debate in the last year has surrounded the adoption of a new set of seventeen Sustainable Development Goals (SDGs) as a guide for low and middle-income countries. Great controversy has surrounded the metrics for the SDGs, with few metrics directly dealing with surgical care, although many of the thirteen targets within the SDG focused on health, require surgical and anesthesia care.

The common deformity of the frst ray known as a “bunion” is a progressive positional deformity which leads to pain from shoe pressure and biomechanical malfunction of the frst metatarsal phalangeal joint. While the medial bump is idely considered the etiology of pain, malalignment results in progressive joint adaptation and degeneration. The exact biomechanical fault and the etiology of the progression of the deformity remain unclear. The origin of the terminologies describing this frst ray deformity deserves specifc attention due to the common historical misapplications of terms used to describe disorders of the frst metatarsophalangeal joint (MTPJ) .Bunion is derived from the Latin term bunio, meaning turnip. This term has been applied to describe any enlargement of the frst MTPJ and therefore poorly defnes the deformity . It was not until 1870, when Carl Hueter, a German surgeon, coined the term hallux valgus to more accurately describe the condition . Hueter defned this frst ray deformity as a subluxation of the frst MTPJ in the transverse plane with lateral deviation of the great toe and medial deviation of the frst metatarsal. However, the term hallux valgus raised questions on whether the laterally deviated hallux should be the primary focus of the deformity. Therefore, half a century later, Truslow proposed the term metatarsus primus varus to replace hallux valgus in the belief that the medially deviated frst metatarsal is the primary level of deformity. This is in fact the frst time the primary level of deformity is considered to be located at the frst metatarsal cuneiform joint.

Closed or endonasal rhinoplasty has been practiced since the dawn of the modern rhinoplasty era, see Roe’s description in 1887 . In recent years there has been a growing enthusiasm for both teaching and practice of the open approach to rhinoplasty; this technique is both easier to teach and for the student to learn; however, there are small but signifcant prices to pay for this choice. The most signifcant is the presence of an external scar on the columella, which proponents of the technique claim to be near invisible but which a signifcant population of patients fnd unsightly .Also, despite the clear visibility of both cartilage and bone in the open technique, there is frequent irregularity of contour and of symmetry, especially in inexperienced hands . The authors prefer the closed technique for its precision, rapid recovery, and lack of external scarring. We also fnd the technique sympathetic to the frequent frustrations of the less experienced surgeon attempting the technique. For that reason we teach it to our fellows as a “beginner’s” technique until suffcient experience has been gained to graduate to the more technically demanding open procedure .

Under the restraint of the title of the book, general symptoms of OSMF, like burning in oral cavity, intolerance to temperature and spicy food, and general nutritional defciency symptoms, are excluded. Authors shall restrict to the pathological changes in oral cavity responsible for trismus. The structures classically affected are the cheek, teeth, anterior tonsillar pillar and soft palate, muscles of mastication, and temporomandibular joint. Hence, the surgeon needs to address the dense hyalinized tissues in all these areas to achieve adequate release of trismus, i.e., to improve mouth opening. The following pictures shall demonstrate all above and the factors that need to be considered before surgery. Grading is done strictly after 3 months of cessation of the injurious habits of chewing tobacco, areca nut, catechu mixed with lime, and similar offending habits. Grades are measured in centimeters of inter-incisor distance (IID). Grade 0: Symptoms of OSMF without trismus Grade 1: Inter-incisor distance equal to or more than 3 cm. Grade 2: Inter-incisor distance between 2.9 and 2 cm. Grade 3: Inter-incisor distance between 1.9 and 1 cm. Grade 4: Inter-incisor distance less than 1 cm.

The inferior vena cava (IVC) is the main vein of the human body, formed by the confluence of the left and right common iliac veins. It ascends in the retroperitoneum to the right of the aorta and exits the abdomen through the diaphragmatic hiatus to join the right atrium. It drains the left and right renal veins, the lumbar veins, the right adrenal vein, the right gonadal vein, and the hepatic veins. The azygos venous system connects to the IVC (directly or through the renal veins). The IVC has four segments: the hepatic, suprarenal, renal, and infrarenal segments . Formation of the IVC is the result of anastomoses and regression of embryonic veins including the vitelline vein and paired posterior cardinal, supracardinal, and subcardinal veins. The hepatic segment is composed of the vitelline vein, the suprarenal segment is composed of a segment of the right subcardinal vein, and the renal egment is formed by the anastomosis between the right subcardinal and the right supracardinal veins; a part of the supracardinal vein constitutes the infrarenal segment . Knowledge of the IVC disease is primordial before surgery to avoid serious complications. This chapter will focus on the imaging techniques, the main diagnostic features, and the interventional radiology of IVC disease.

Optimizing Outcomes for Liver and Pancreas Surgery

In the United States, as the large baby boomer population cohort continues to age, the number of patients presenting with hepatopancreatobiliary (HPB) malignancies has and will continue to rise into the foreseeable future. The US Census Bureau projects that the number of adults aged 65 and older is expected to increase from 46 million in 2014, to 74 million by 2030 . The median age for cancer diagnosis is 66 years, making advanced age an increased risk factor for development of HPB carcinomas . In agreement, cancer incidences for both liver and pancreatic cancers are expected to increase from 2010 to 2030 by 59% (liver) and 55% (pancreas), respectively . Thus, in the upcoming decades, substantial healthcare resources and attention will be devoted to treating HPB malignancies. Surgical resection continues to remain the preferred curative treatment option for HPB neoplasms. However, older patients with HPB disease are often frail and have multiple comorbidities alongside their primary malignancy, thus making aggressive surgical resections high risk for these patients. For frail and elderly patients undergoing elective procedures, perioperative care must be afforded special attention in order to decrease incidences of severe morbidity and mortality. While postoperative care is a mainstay of focus for surgical patients, preoperative assessment is often afforded less attention within the feld of HPB surgery. By the use of standardized patient assessments, clinicians are able to obtain a more accurate representation of a patient’s true health status, thus making it possible to identify surgical patient populations with higher risks of postoperative morbidity, mortality, increased length of hospital stay, and increased risk of being discharged to skilled nursing facilities.

Cancer is constituted by mutated cells that have escaped the normal checks and balances of regulated cell growth. It is initially localized to the organ of origin and thereafter spreads through the body, and ultimately becomes the cause of unnatural death of the patient. In the natural history of any cancer three distinct phases can be described: (i) limited to the organ of origin (localized phase); (ii) limited to the region of origin (regional phase); and (iii) spread to distant organs (metastatic phase) . Regional phase/stage is usually described as spread limited to the regional lymph node basin. The philosophy of surgical lymphadenectomy, along with extirpation of the primary tumour, is meant to treat the regional stage of the disease. It will be obvious to those familiar with this feld, that high quality evidence to evaluate lymphadenectomy in oesophageal carcinoma is diffcult to come by, else there would be no need for reviews like this. To the GI Surgery fellows taking their fnal examinations this represents a treacherous minefeld due to the varying practices followed by surgeons across the world. In this review we attempt to introduce the reader to the concept, guidelines, evidence and practice of lymphadenectomy in the surgical treatment of oesophageal carcinoma.

Reoperative Parathyroid Surgery

As with most surgical procedures, a surgeon’s knowledge of the relevant anatomy is critical to the technical success of operations on the parathyroid glands. This knowledge must encompass more than just an understanding of the anatomy of the normal neck; it must also include an understanding of the variant anatomy and aberrant anatomic relationships that may be encountered. The process is reinforced by an appreciation and understanding of not only parathyroid anatomy but also parathyroid embryology. This chapter reviews the embryology and anatomy of the parathyroid glands in a clinically relevant manner. Furthermore, it details recent advances in our understanding of the molecular mechanisms of parathyroid development. Our aim is to reinforce the link between embryology and surgical anatomy in the context of parathyroid surgery such that parathyroid glands are successfully identifed and operative morbidity is minimized. We also intend for this to serve as a reference and preface to the other chapters in this text.

Obesity is defned as an abnormal or excessive accumulation of fat that may impair health, and it is a chronic disease that is increasing in prevalence . Global obesity rates have tripled in many countries of the World Health Organization (WHO) European Region since the 1980s, and the numbers of those affected continue to rise at an alarming rate . Based on the latest estimates in European Union countries, overweight affects 30–70%, and obesity affects 10–30% of adults. In the USA 70% of the population are now affected by excess weight or obesity . It is now no exaggeration to state that obesity is an international epidemic. Moreover, it is no longer a disorder of the adult since obesity prevalence in children has accelerated rapidly affecting 21.1% of girls and 18.6% of European boys (Ahrens et al. 2014).
