ABSTRACT

Bronchial asthma is now widely recognized as a heterogeneous clinical syndrome consisting of various disease phenotypes. Each asthma phenotype may have distinct observable molecular, cellular, morphological,   functional, and clinical features , all of which can be possibly integrated into specifc biological mechanisms, called as endotypes . Although differentiating asthma into various phenotypes/endotypes remains speculative so far, these concepts of separation may be useful in characterizing and predicting disease severity,  progression, and response to general and specifc therapies including biologic medications . This is particularly important for severe asthma patients who are refractory to current standard therapies including inhaled and systemic corticosteroids (CS) and bronchodilators. Because these patients account for a signifcant  proportion of health-care expenditure of asthma , recognizing the heterogeneous nature of asthma,  especially severe asthma, may enable us to develop safe and effective phenotype-targeted biological  therapies.

INTRODUCTION

Importantly, appropriate clinical phenotyping of severe asthma patients, in turn, inevitably requires  standardized defnition of severe asthma which can be applied to a wide range of populations all over the  world. There have been numerous proposed defnitions for severe asthma in association with several  respiratory and medical societies. It has been also referred as diffcult, therapy-resistant, as well as refractory asthma. Firstly, to properly defne the clinical situation of severe asthma, a prior diagnosis of asthma should be made. Then, clinical symptoms of bronchial asthma should persist despite the maximal treatment of current therapies. In general, previous studies have suggested that failure of controlling asthma symptoms despite the prescription of high-dose inhaled corticosteroids (ICS) may be a minimum requirement of defnition for  severe asthma, and numerous recent works have also stipulated the therapeutic level of severe asthma as those equivalent to high-dose therapies  (see Table 1.1).  The frst defnitions of severe asthma were proposed in 1999 and in 2000 by European Respiratory Society (ERS)  and American Thoracic Society (ATS) ,  respectively (see Table 1.1).

آسم برونش در حال حاضر به طور گسترده ای به عنوان یک سندرم بالینی ناهمگن متشکل از فنوتیپ های مختلف بیماری شناخته می شود. هر فنوتيپ آسم ممکن است داراي ويژگي هاي مولکولي، سلولي، مورفولوژيکي، عملکردي و باليني قابل مشاهده باشد که مي توان آنها را به مکانيسم هاي بيولوژيک خاصي که به عنوان اندوتيپ ها ناميده مي شوند، پيوند دهند. اگر چه تمایز آسم به فنوتایپ ها / اندوتایپ های مختلف تا کنون به نظر می رسد، این مفاهیم تفکیک ممکن است در توصیف و پیش بینی شدت بیماری، پیشرفت و پاسخ به درمان های عمومی و خاص از جمله داروهای بیولوژیک مفید باشد. این به خصوص برای بیماران مبتلا به آسم بسیار مضر است و به درمانهای استاندارد فعلی از جمله کورتیکواستروئیدهای استنشاقی و سیستمیک (CS) و برونکودیلاتورها مقاوم است. از آنجا که این بیماران بخش قابل توجهی از هزینه های مراقبت های بهداشتی آسم را تشکیل می دهند، شناخت ماهیت ناهمگونی آسم، به ویژه آسم شدید، ما را قادر می سازد تا روش های بیولوژیکی ایمن و موثر فنوتیپ هدفمند را توسعه دهیم.

مقدمه

مهمتر از آن، فنوتیپ بالینی مناسب بیماران مبتلا به آسم، به ناچار نیازمند تعریف استاندارد از آسم شدید است که می تواند به طیف وسیعی از جمعیت در سراسر جهان اعمال شود. در ارتباط با چندین جوامع تنفسی و پزشکی بسیاری از آلودگی های شدید آسم وجود دارد. همچنین به عنوان درمان آکنه مقاوم، مقاوم در برابر درمان و همچنین آفت کش مقاوم به نظر می رسد. در مرحله اول، به درستی وضعیت بالینی آسم شدید، تشخیص قبلی آسم باید انجام شود. سپس علائم بالینی آسم برونشال باقی می ماند با وجود درمان حداکثر درمان جاری. به طور کلی، مطالعات قبلی پیشنهاد کرده اند که عدم کنترل علائم آسم با وجود تجویز کورتیکواستروئیدهای استنشاقی با دوز بالا (ICS) ممکن است حداقل نیاز به تعویض برای آسم شدید باشد، و بسیاری از آثار اخیر نیز میزان درمانی آسم شدید را تعیین کرده اند کسانی که معادل داروهای با دوز بالا هستند (جدول 1.1 را ببینید). در سال 1999 و در سال 2000 توسط انجمن های تنفسی اروپایی (ERS) و انجمن توراکسای آمریکایی (ATS) پیشگیری از آسم شدید (به جدول 1.1 ارائه شد).
