Pulmonary Vasculature Redox Signaling in Health and Disease

ABSTRACT

Pulmonary arterial hypertension (PAH) is a progressive disease of the lung vasculature, which is characterized by sustained pulmonary arterial pressure, resulting in increased pulmonary vascular resistance, with eventual right heart failure . Vascular remodeling caused by the medial hyperplasia of pulmonary artery (PA) smooth muscle cells is a hallmark feature of PAH , which causes occlusion of the vessels . In most forms of PAH, muscularization of small distal PA occurs , and is further characterized by excessive vascular cell proliferation, inward remodeling, rarefaction, and a loss of compliance of the pulmonary blood vessels . Increased  resistance to blood flow and more rigid blood vessels (loss of vascular compliance) leads to failure of the right ventricle and eventual death. PAH is more frequent in women than men, and left  untreated has a survival time of 5–7 years post diagnosis . From a therapeutic standpoint, there are a number of vasodilator drugs that are indicated for the treatment of PAH, but none of the current therapeutics offers long-term success for survival due to limited effectiveness and unwanted side effects , and more importantly, do not address the underlying causes of the disease.

INTRODUCTION

The generation of reactive oxygen species (ROS) is well recognized as a pathophysiological mechanism underlying the vascular remodeling and proliferation that occurs in PAH. The major ROS that are produced in the pulmonary vasculature are superoxide (O2), hydrogen peroxide (H2O2), hydroxyl radical (OH.), and hydroperoxyl radical (HO˙2) . Of these ROS, both O2and H2O2 activate multiple signaling pathways that cause cell proliferation and apoptosis, elevated vascular tone, fbrosis, and inflammation, which are all hallmark signs of PAH . However, the cellular origin and functional signifcance of ROS in PAH remain poorly delineated. Elevated levels of ROS in PAH occur because there is increased production and decreased enzymatic  degradation of the ROS moieties, of which, evidence exists for both phenomena in the underlying etiology of elevated  pulmonary arterial pressure and pulmonary arterial resistance.

چکیده

پرفشاری خون شریانی ریوی (PAH) یک بیماری پیشرونده عروق ریوی است که با فشار شریان ریوی پایدار مشخص شده و منجر به افزایش مقاومت عروقی ریوی و نارسایی احتمالی قلب می شود. بازسازی عروق ناشی از هیپرپلازی Medial of Arthroponic Artery (PA) سلول های عضلانی صاف، یکی از ویژگی های مشخص PAH است که باعث انسداد عروق می شود. در اکثر اشکال PAH، عضلانی کردن پالس ديستال کوچک رخ می دهد و بیشتر با تکثیر سلولی عروقی، بازسازی درونی، رفلاکس و عدم رعایت رگ های خونی ریه مشخص می شود. افزایش مقاومت به جریان خون و عروق خونی عضلانی (از دست دادن رعایت عروق) منجر به شکست بطن راست و مرگ ناگهانی می شود. PAH در زنان بیش از مردان بیشتر است و درمان بدون درمان دارای زمان بقاء 5-7 سال پس از تشخیص است. از دیدگاه درمانی، تعدادی از داروهای وازودیلاتور وجود دارد که برای درمان PAH وجود دارد، اما هیچ یک از درمان های جاری برای موفقیت درازمدت برای بقا به علت اثربخشی محدود و عوارض جانبی ناخواسته، و از همه مهمتر، نشانی از علل زمينه اي بيماري.

مقدمه

نسل از گونه های اکسیژن واکنشی (ROS) به عنوان یک مکانیزم پاتوفیزیولوژیک زیرمجموعه بازسازی و تکثیر عروقی که در PAH رخ می دهد، به رسمیت شناخته شده است. ROS عمده ای که در عروق ریوی تولید می شوند، عبارتند از سوپر اکسید (O2-)، پراکسید هیدروژن (H2O2)، هیدروکسیل رادیال (OH.) و رادیکال هیدروپروکسیل (HO˙2). از این ROS، هر دو O2- و H2O2 چندین مسیر سیگنالینگ را فعال می کنند که باعث تکثیر و آپوپتوز سلولی، افزایش سطح خون عروق، فیبروز و سرگیجه می شود که همه نشانه های مشخص PAH هستند. با این حال، منشاء سلولی و نشانه های عملکردی ROS در PAH هنوز ضعیف مشخص نشده اند. سطح بالای ROS در PAH رخ می دهد به دلیل افزایش تولید و کاهش تجزیه آنزیمی ROS، که شواهدی وجود دارد برای هر دو پدیده در علل زیربنایی افزایش فشار خون ریوی و مقاومت در برابر شریان ریوی وجود دارد.
