ABSTRACT
A wide variety of lesions can be found in the vertebral column. The lesions can be solitary or multiple and include both benign and malignant lesions. The differential diagnosis of a vertebral tumour can be narrowed down based on the age of the patient, the number of lesions, the location in the vertebra, the location in the spinal column, the morphology and the imaging characteristics (Algorithms 1 and 2). Initial imaging usually consists of plain radiography. This is not very sensitive mainly because of the poor two-dimensional tissue separation due to the complex three-dimensional anatomy of the spine. The sensitivity to specify a vertebral lesion on an X-ray is diffcult as well. Magnetic resonance imaging (MRI) is the imaging modality of choice when it comes to detecting and characterizing tumours of the osseous spine. MRI has the advantage of superior soft tissue characterization, the possibility of multiplanar imaging and the ability of evaluating present neurological compromise. Computed tomography (CT) is superior in detecting calcifcations or cortical bone lesions. Since this technique offers multiplanar imaging too, it is very well suited to evaluate the complex anatomy of the posterior vertebral elements and assess some typical lesions in this anatomical region. Furthermore CT plays an important role in biopsy guidance or therapeutic ablation of certain tumours.
INTRODUCTION
The patient’s age is a very important piece of information when it comes to differentiating a vertebral tumour (Algorithm 2). For example, osteoid osteoma, aneurysmal bone cyst (ABC) and Ewing sarcoma are predominantly found in young people,giant cell tumour (GCT) in the middle-aged and chordoma in the elderly. The value of a thorough clinical history and examination should not be underestimated. Multiple vertebral body lesions in a patient with a known primary tumour will most likely be metastases. Depending on the extent and location of the tumour, clinical symptoms may arise. Osteoid osteoma, aggressive haemangioma and malignancies are typically symptomatic.
چکیده
طیف وسیعی از ضایعات را می توان در ستون مهره یافت. ضایعات می توانند انحصاری یا چندگانه باشند و شامل ضایعات خوش خیم و بدخیم باشند. تشخیص افتراقی تومور مهره ای می تواند بر اساس سن بیمار، تعداد ضایعات، محل در مهره، محل ستون فقرات، مورفولوژی و ویژگی های تصویربرداری (الگوریتم 1 و 2) محدود شود. تصویربرداری اولیه معمولا شامل رادیوگرافی ساده است. این به دلیل جدا شدن بافت ضعیف دو بعدی به علت آناتومی پیچیده سه بعدی ستون فقرات، بسیار حساس نیست. حساسیت برای مشخص نمودن ضایعه مهره ای روی اشعه ایکس، همچنین دشوار است. تصویربرداری رزونانس مغناطیسی (MRI) روش انتخاب تصویربرداری انتخابی است که به تشخیص و تشخیص تومورهای ستون فقرات استخوانی می پردازد.
