ABSTRACT
Prostate cancer is characterized by a low mortality—incidence ratio partially due to the diagnosis of localized and low-risk disease . Focal therapy for prostate cancer has evolved in order to address the need for treatment of localized disease without exposing patients to the morbidities associated with radical therapy such as incontinence and erectile dysfunction and thus allowing them to maintain a good quality of life. In women, similarly to prostate cancer, the overdiagnosis of breast cancer due to improved imaging has induced the progression of surgical techniques from radical mastectomy to a more focal approach with individualized breast conservative strategies. In the following chapter, we will be reviewing the evolution of focal therapy in breast and prostate cancer and will examine what were the factors that drove this evolution in time.
INTRODUCTION
As the leading cause of cancer death in women all over the world, breast cancer is the most frequently diagnosed. It represents a growing and challenging problem for health services; currently its high incidence and mortality is considered a global public health problem . It is also a systemic disease, with a heterogeneous behavior. It is mainly due to the tumors’ biological characteristics , which are defined by different genetic profiles or immunohistochemistry. These characteristics are critical in determining the prognostic and predictive factors in each case. In 1894 William Halsted published the results of his study “The results of operations for the cure of cancer of the breast performed at the Johns Hopkins Hospital from June 1889 to January 1894” . In this study, a total of 50 patients underwent excision en bloc of the mammary gland, both pectoral muscles, and axillary lymph nodes. The procedure was justified on the Halsted theory , which argues that the tumor extends orderly and predictably, based on mechanical considerations. It also contemplates that the disease develops in stages: in the first one, the cancer develops in the organ in which it appears; the second regional stage in which it spreads to regional lymph nodes draining that circulation lymphatic; and finally, the systemic phase, which explains the metastasis. In this study there were no operative deaths, the local recurrence rate was 6 % (3 patients), and the 3-year survival rate was 45 % .
چکیده
سرطان پروستات به علت تشخیص بیماری های موضعی و کم خطر، تقریبا از میزان مرگ و میر کم است. درمان کانونی برای سرطان پروستات به منظور رسیدگی به نیاز به درمان بیماری موضعی بدون ارائه بیماران به بیماری های مرتبط با درمان رادیکال مانند بی اختیاری و اختلال نعوظ و به این ترتیب اجازه می دهد آنها را برای حفظ کیفیت خوب زندگی تکامل یافته است. در زنان، همانند سرطان پروستات، تشخیص بیش از حد از سرطان پستان با توجه به بهبود تصویربرداری، پیشرفت تکنیک های جراحی را از ماستکتومی رادیکال به یک رویکرد کانونی با استراتژی محافظه کارانه فردی است. در فصل بعد ما بررسی تکامل درمان کانونی در سرطان سینه و پروستات را بررسی خواهیم کرد و عوامل موثر بر این تکامل را در زمان بررسی خواهد کرد.
مقدمه
به عنوان عامل اصلی مرگ و میر ناشی از سرطان در زنان در سراسر جهان، سرطان پستان اغلب تشخیص داده می شود. این نشان دهنده یک مشکل رو به رشد و چالش برانگیز برای خدمات بهداشتی است؛ در حال حاضر بروز بالا و مرگ و میر آن یک مشکل جهانی سلامت عمومی است. این یک بیماری سیستمیک است، با رفتار ناهمگون. این عمدتا به دلیل ویژگی های زیستی تومورها است که توسط پروفیل های ژنتیکی مختلف یا ایمونوهیستوشیمی تعریف شده است. این ویژگی ها در تعیین عوامل پیش آگهی و پیش بینی در هر مورد مهم هستند. در سال 1894 ویلیام هالست منتشر نتایج مطالعه خود “نتایج عملیات برای درمان سرطان سینه انجام شده در بیمارستان جان هاپکینز از ژوئن 1889 تا ژانویه 1894”. در این مطالعه 50 بیمار تحت عمل جراحی برداشتن غده پستان، هر دو عضلات گردن و غدد لنفاوی زیر بغل قرار گرفتند. این روش در نظریه هالستد توجیه می شود که بیان می کند که تومور به طور منظم و قابل پیش بینی بر اساس ملاحظات مکانیکی گسترش می یابد. همچنین در نظر دارد که بیماری در مراحل مختلف رشد کند: در ابتدا، سرطان در اندام ایجاد می شود؛ مرحله دوم منطقه ای که در آن به غدد لنفاوی منطقه تخلیه آن لنفاوی گردش خون گسترش می یابد؛ و در نهایت، فاز سیستمیک، که متاستاز را توضیح می دهد. در این مطالعه هیچ مرگ و میر ناشی از عمل وجود نداشت، میزان عود مجدد 6٪ (3 بیمار) و میزان بقاء 3 ساله 45٪ بود.
