Ocular Tuberculosis

ABSTRACT

Tuberculosis (TB) is an airborne infection caused by Mycobacterium tuberculosis. While 10% of individuals with infected TB become symptomatic, in about 90% the infection remains latent without manifestation of the disease for the rest of their lives. In 1999, the World Health Organization (WHO) declared tuberculosis a global emergency causing nearly three million deaths annually. In the same year, WHO reported that there were 3.7 million cases of tuberculosis of which 20% of patients came from Africa, 38% from Southeast Asia, 22% from Western Pacifc, 6% from the United States, and 4% from the eastern Mediterranean.

INTRODUCTION

The incidence of TB in developed countries decreased in the nineteenth century. This decline was attributed to improved living conditions and the discovery of effective antibiotics. In developed countries, such as the United States, the prevalence of ocular TB is estimated to be around 1–2%, while in Spain it is signifcantly higher at 18% (see Table 1.1). In contrast in developing countries, there are approximately 8.4 million new cases of TB annually with a corresponding increased incidence of ocular TB . Ocular TB was frst diagnosed by Maitre-Jan in 1711. The current prevalence of ocular TB varies from 0.5% to 1.4% . Ocular TB is defned as an infection by M. tuberculosis which can involve any part of the eye, superfcial, intraocular, or the structures around the eye with or without systemic involvement . In primary ocular TB which is rare, the eye is the portal of entry which may be the conjunctiva, cornea, or sclera. In secondary ocular TB, the ocular involvement occurs by hematogenous spread of the organism, of which tuberculous uveitis is an example. Due to the rich blood supply of the uveal tract, the choroid is the most common site of ocular TB manifestation . Ocular TB often presents without clinical evidence of active pulmonary TB and may be the
frst and only manifestation of the infection
. Among the various forms of ocular TB, posterior uveitis is the most common.

چکیده

Tuberculosis (TB) یک عفونت هوایی ناشی از Mycobacterium tuberculosis است. در حالی که 10٪ از افراد مبتلا به TB آلوده به علامت هستند، در حدود 90٪ عفونت باقی می ماند بدون تظاهرات بیماری برای بقیه زندگی خود. در سال 1999، سازمان بهداشت جهانی (WHO) سل را به عنوان یک اورژانس جهانی اعلام کرد که سالانه حدود 3 میلیون مرگ و میر ناشی از آن را تشکیل می دهد. در همان سال، WHO گزارش داد که 3.7 میلیون مورد سل وجود دارد که 20٪ از آنها از آفریقا، 38٪ از جنوب شرقی آسیا، 22٪ از Western Pacifc، 6٪ از ایالات متحده و 4٪ از شرق مدیترانه ای است.

مقدمه

شیوع سل در کشورهای توسعه یافته در قرن نوزدهم کاهش یافته است. این کاهش ناشی از بهبود شرایط زندگی و کشف آنتی بیوتیک های موثر بود. در کشورهای توسعه یافته مانند ایالات متحده، شیوع TB چشم حدود 1-2٪ تخمین زده می شود، در حالی که در اسپانیا، آن به میزان قابل توجهی بالاتر از 18 درصد است (جدول 1.1). در مقابل، در کشورهای در حال توسعه، سالانه حدود 8.4 میلیون مورد جدید سل با وجود افزایش بالایی از سلولهای TB دیده می شود. شایعترین شیوع سل ریوی از 0.5 تا 1.4 درصد است. TB چشم به عنوان یک عفونت توسط M. tuberculosis تعریف شده است که می تواند هر بخشی از چشم، فوق فکی، داخل چشم و یا ساختارهای اطراف چشم با یا بدون دخالت سیستمیک را شامل شود. در TB چشم اولیه که نادر است، چشم پورتال ورودی است که ممکن است conjunctiva، cornea یا sclera باشد. در سلول TB چشم دوم، درگیری های چشم به وسیله انتشار هماتوژن ارگانیسم اتفاق می افتد که نمونه ای از یوئیت توبرکلوزی است. به علت تامین خون غنی از دستگاه مجرای اوائل، کرویید شایع ترین محل تظاهرات سلول های TB چشم است. TB چشم اغلب بدون شواهد بالینی از سل ریوی فعال وجود دارد و ممکن است برست و تنها تظاهر عفونت است. از جمله اشکال مختلف سل ریوی، uveitis خلفی شایع ترین است.
