ABSTRACT

Cancer is constituted by mutated cells that have escaped the normal checks and balances of regulated cell growth. It is initially localized to the organ of origin and thereafter spreads through the body, and ultimately becomes the cause of unnatural death of the patient. In the natural history of any cancer three distinct phases can be described: (i) limited to the organ of origin (localized phase); (ii) limited to the region of origin (regional phase); and (iii) spread to distant organs (metastatic phase) . Regional phase/stage is usually described as spread limited to the regional lymph node basin. The philosophy of surgical lymphadenectomy, along with extirpation of the primary tumour, is meant to treat the regional stage of the disease. It will be obvious to those familiar with this feld, that high quality evidence to evaluate lymphadenectomy in oesophageal carcinoma is diffcult to come by, else there would be no need for reviews like this. To the GI Surgery fellows taking their fnal examinations this represents a treacherous minefeld due to the varying practices followed by surgeons across the world. In this review we attempt to introduce the reader to the concept, guidelines, evidence and practice of lymphadenectomy in the surgical treatment of oesophageal carcinoma.

INTRODUCTION

William Halstead’s work on breast cancer at the turn of the 20th century laid the foundation of the ‘Halstedian’ philosophy of growth of cancer. This was founded on the concept that cancer sequentially grows outward from the primary lesion to involve the lymph nodes, and then from the lymph nodes, the metastatic cells enter the bloodstream and lead to distant metastases. Based on this principle radical mastectomy was espoused to cure breast cancer in the ‘lymph nodal’ stage of the disease. About a century later, data from other cancers seems to agree with this philosophy. Gastric cancer data from the National Cancer Centre, Tokyo (Table 1.1), shows the sequential early, intermediate and late stage of the disease . This table shows that lymph nodal spread increases in the intermediate stages and distant spread increases in the late stages.

چکیده

سرطان توسط سلول های جهش یافته ای ساخته شده است که از کنترل طبیعی و تعادل رشد سلول های تنظیم شده جلوگیری می کند. این در ابتدا به ارگان مبدأ منتقل شده و پس از آن از طریق بدن گسترش می یابد و در نهایت باعث مرگ غیرطبیعی بیمار می شود. در تاریخ طبیعی هر سرطان سه مرحله مجزا را می توان شرح داد: (1) محدود به ارگان مبدا (فاز محلی)؛ (ii) محدود به منطقه مبدأ (مرحله منطقه ای)؛ و (iii) گسترش به اندام های دور (مرحله متاستاتیک). فاز / مرحله منطقه معمولا به عنوان محدود به حوزه گره لنفاوی منطقه شناخته شده است. فلسفه لنفادنکتومی جراحی همراه با برداشتن تومور اولیه، به منظور درمان مرحله منطقه ای بیماری است. برای افرادی که با این فلاش آشنا هستند، واضح است که شواهد با کیفیت بالا برای ارزیابی لنفادنکتومی در کارسینوم ورزشکار دچار مشکل می شوند؛ در غیر اینصورت نیازی به بررسی هایی مانند این نیست. به همراهان جراحی جراحی، با انجام معاینات جراحی، این نشان دهنده یک معدن خائنانه به علت شیوه های مختلف جراحان جراحی در سراسر جهان است. در این بررسی ما تلاش می کنیم خواننده را به مفهوم، رهنمودها، شواهد و عملکرد لنفادنکتومی در درمان جراحی کارسینومای کاردیو ارئه کنیم.

مقدمه

کار ویلیام هالستد در زمینه سرطان سینه در اواسط قرن بیستم، پایه و اساس فلسفه هالستیان رشد سرطان را به وجود آورد. این بر اساس این مفهوم بود که سرطان به طور متوالی از ضایعه اولیه به وجود می آورد تا گره های لنفاوی را به وجود آورد، و سپس از گره های لنفاوی، سلول های متاستاتیک وارد جریان خون می شوند و منجر به متاستاز های دور می شوند. بر اساس این اصل، ماستکتومی رادیکال برای درمان سرطان پستان در مرحله لنفاوی بیماری لوزالمعده حمایت شد. حدود یک قرن بعد، داده های مربوط به سرطان های دیگر به نظر می رسد با این فلسفه موافق هستند. داده های سرطان معده از مرکز ملی سرطان، توکیو (جدول 1.1)، مراحل متوالی زودرس، میانی و اواخر بیماری را نشان می دهد. این جدول نشان می دهد که گسترش نود لنفاوی در مراحل متوسط و گسترش گسترش دوردست در مراحل اواخر افزایش می یابد.
