Dermal Drug Selection and Development

ABSTRACT

In this chapter, the manner a new drug is discovered and selected as well as the costs, challenges and risks around the selection of a candidate molecule are discussed. As candidate quality impacts attrition in later phases of development this concept is developed as well. The aim is to frame the key aspects of drug candidate selection in general to place into context the challenges that a drug discovery team would face for the selection of a dermal drug candidate. Finally, specifc risks and opportunities for selecting such a dermal drug candidate are presented.

INTRODUCTION

Developing a new drug, whether a small or large molecule, whether for local delivery or systemic delivery, is a complex process which can take 12–15 years and cost in excess of $1 billion . The process to develop a new drug is divided in several steps and mainly aims at minimizing the substantial risks inherent to such a development. It follows broadly fve stages as shown in Fig. 1.1. The frst is a research stage during which the target, the mechanism of action is considered and studied. Such research often occurs in academia. Once the mechanism of action is associated with a disease, a second stage can begin where a pharmaceutical/biotech company could decide to start a drug discovery project aiming at fnding a candidate molecule. If the candidate molecule is found, the project will move into a preclinical development stage where various efforts especially in safety assessment, pharmaceutics development and chemical development will occur. Then the clinical phases will start and eventually, if successful, a New Drug Application will be fled to regulatory bodies. Within this process, drug discovery (lead discovery in the previous fgure) is the stage during which the drug molecule will be designed, synthesised, assessed and selected. It will involve efforts from disciplines such as biology (in vitro and in vivo pharmacology), medicinal chemistry, pharmacokinetics and safety assessment. Many assays will be performed during this drug discovery phase (Fig. 1.2).

چکیده

در این فصل، شیوه ای که داروی جدید کشف و انتخاب می شود و همچنین هزینه ها، چالش ها و خطرات انتخاب مولکول نامزد مورد بحث قرار گرفته است. این مفهوم به عنوان کیفیت کاندیدایی در دوره های بعد توسعه تاثیر می گذارد. هدف این است که چارچوب جنبه های کلیدی انتخاب نام دارو را به طور کلی به چالش بکشیم که چالش هایی که یک تیم کشف مواد مخدر برای انتخاب یک نامزد مواد مخدر پوستی ایجاد می کند. در نهایت، ریسک های خاص و فرصت های انتخاب چنین کاندیدای مواد مخدر پوست ارائه می شود.

مقدمه

توسعه یک داروی جدید، چه مولکولی کوچک یا بزرگ، چه برای تحویل محلی یا تحویل سیستمیک، فرایندی پیچیده است که می تواند 12-15 سال طول بکشد و هزینه بیش از 1 میلیارد دلار باشد. فرآیند توسعه یک داروی جدید به چندین مرحله تقسیم می شود و عمدتا با هدف به حداقل رساندن ریسک های قابل توجهی که برای چنین توسعه ای وجود دارد، می باشد. این به طور گسترده ای مراحل زیر را دنبال می کند که در شکل 1.1 نشان داده شده است. فرآیند یک مرحله تحقیق است که طی آن هدف، مکانیسم عمل در نظر گرفته می شود و مورد مطالعه قرار می گیرد. چنین تحقیقاتی اغلب در دانشگاه ها رخ می دهد. هنگامی که مکانیسم عمل با یک بیماری مرتبط است، مرحله دوم می تواند شروع شود که یک شرکت دارویی / بیوتکنولوژی می تواند تصمیم به شروع یک پروژه کشف مواد مخدر با هدف ایجاد یک مولکول نامزد داشته باشد. اگر مولکول کاندید یافت شود، این پروژه به مرحله تکامل دلیلی حرکت خواهد کرد که در آن تلاش های مختلف به ویژه در ارزیابی ایمنی، توسعه دارو و توسعه شیمیایی رخ خواهد داد. سپس مراحل بالینی آغاز می شود و در نهایت، اگر موفق شود، یک برنامه جدید برای فرار از بدن کنترل می شود. در این فرآیند، کشف مواد مخدر (کشف سرب در فگور قبلی) مرحله ای است که طی آن مولکول دارو طراحی و سنتز می شود، ارزیابی و انتخاب می شود. این شامل تلاش هایی از رشته هایی مانند زیست شناسی (در آزمایشگاهی و درونمایه فارماکولوژی)، شیمی دارویی، فارماکوکینتیک و ارزیابی ایمنی خواهد بود. بسیاری از آزمایش ها در طی این مرحله کشف مواد مخدر انجام می شود (شکل 1.2).
